My phone stopped vibrating - with everything.  calls, key pad, texts...all notifications.  Took it into Verizon store and they checked all setttings and was good.  Said to do the upgrade...did that, but vibrate still not working.  Next step is replacement. Am I missing something?

Sounds like the motor for the vibration died. If verizon didnt find an issue with settings then it seems a new phone is needed.

Just make sure all items that you want to vibrate are set with the proper settings if this wasnt done already. im assuming if you went to verizon, then it was..

I concur with JRocker. If Verizon says the settings are OK and it still won't vibrate it sounds like mechanical failure. Verizon should issue you a new phone.

I actually ran into this when I was on an earlier version of the leak, everything was fine then out of the blue it stopped vibrating until I upgraded to version three. I'm assuming they had you go to the OTA, but it could very well be a firmware issue as well as a bad motor. You could try a factory reset before taking your phone in to get it replaced, just remember to back up all of your important things because anything not stored on your sd card will be wiped.
